*Annual Health and Medical Forms part a and b along with a copy of your insurance medical card should be submitted either by mail or by hand to the Stockton Scout Service Center: 1545 St. Mark's Plaza, Ste #7  Stockton  95207  or  faxed to (209)954-9207.

Anyone attending camp (adult leader volunteers, cub scouts, youth helpers/den chiefs and tot-lot siblings) must submit medical forms.

Early Discount
Before 5/30/2015 a discount of $10.00 will apply to all Bear Cub Scout Registrants.

Late fee
After 6/28/2015 a fee of $10.00 will apply to all Bear Cub Scout Registrants.

Cancellation Policy
All fees are non-refundable with the exception of summer school, a death in the immediate family, or a doctor's excused illness in which all but $35 will be refunded. Requests must be submitted in writing with appropriate documentation (i.e.: doctor's excuse or summer school enrollment verification). Refund requests can be submitted at any time and no refund requests will be awarded after September 1, 2015.
